Applicants will only be accepted to a program once ALL of the following steps have been completed:

• Complete the online application
• Submit a photo (headshot) of yourself by email to admissions@livnot.com
• Scan/email or fax your signed waiver form
• Scan/email or fax your completed medical form (we can accept your medical form from other Israel programs, but they do not share your info with us—you must personally retrieve and send the form)
• Scan/email or fax your completed therapist referral form (only if requested after phone interview)
• Submit your deposit—this holds your spot!
• Schedule a phone interview (our office will contact you to schedule a time)
